To be eligible to practice and compete in sports and some CHSAA activites, families need to create one parent and one student account with PlanetHS at www.planeths.com, fill out all required forms online, and upload a Physical Evaluation stating they are cleared for activity, signed by a licensed Physician (these are available at PlanetHS once you've signed up). 

We've provided the form below, but you are still required to complete all forms online at PlanetHS.
