If you are a new student in Archuleta School District 50 Joint, please print and fill out the attached Enrollment Packet. The following forms are included:
- Demographic Information
- Request for records from the student's previous school (fill out top half only)
- Home Language Survey
- Health Questionnaire
- State of Colorado Immunization Policy
- Homeless Survey
- Colorado MEP Occupational Survey
- Student Contract for Internet Use (student and parent signature required)
- Use of Photographs
- Student Records/Release of Information on Students
- Military Opt-Out
- Bus Rules and Contract
Please bring the student's birth certificate and copy of immunization records in addition to the completed Enrollment Packet.
If you qualify to receive free and reduced price school meals, please print and fill out the attached FRAM Packet. Guidelines for qualification are listed on the first page of the packet.
