If your student has health needs such as Asthma, Anaphylactic Allergies, Diabetes, Food Allergies, or other Medication needs that will require attention during the 2024-2025 school year, they will need doctor's orders on file.

 

Also, if you choose to exempt your student from certain immunizations, please remember that these exemptions must be completed every year. Below, we have linked some common healthcare plan forms and the immunization exemption module for your access.
