New Student Enrollment 2023-24

If you are a new student in Archuleta School District 50 Joint, please print and fill out the attached Enrollment Packet. The following forms are included:
- Demographic Information
- Request for records from the student's previous school
- Home Language Survey
- Health Questionnaire
- State of Colorado Immunization Policy
- Homeless Survey
- Colorado MEP Occupational Survey
- Student Contract for Internet Use (student and parent signature required)
- Use of Photographs
- Military Opt-Out
- Free and Reduced Meal Applications
Please bring the student's birth certificate and a copy of immunization records in addition to the completed Enrollment Packet.
