We will be having a FREE dental clinic at the Pagosa Springs High School on November 30. Please see the attached letter and form. All students can participate with parent consent.
To enroll your child in the clinic, we have three options:

Scan the QR code on the attached letter and complete the online form

Complete the attached form and return it to the school (or email to me)

Get paper forms from the school front office (970-264-2231), complete and return to the school (or email to me)
Forms must be turned in by Wednesday, November 8th.
Dates of the clinics are subject to change.
